How do you add shipping insurance to Squarespace Commerce?

For reference, Squarespace has no public checkout-fee endpoint, so one common way to present an opt-in offer is an optional "Shipping insurance" item the shopper adds on purpose. It should never be added to the cart automatically. Its description should say that it is shipping insurance, who insures the shipment, and link the terms. Exclude it from discounts and bundles where you can, so its price stays clear.
